1. What is your age, height, and weight?

2. How long have you been an endurance athlete? Please describe your background in detail. If you have racing experience, list results and explain.

3. Describe your riding style. Are you a climber? Time-trialist? Sprinter? Do you push big gears or prefer to spin at a high cadence? Do you stand a lot on climbs?

4. Have you ever trained with a heart-rate monitor or power meter?

5. What model bike do you have? Were you fitted?

6. Are you a member of a gym? Do you have access to a gym? If not, what do you have at home to work the legs and core? Weights, squat rack, leg press, dumbbells, ankle weights, exercise ball? Please give details.

7. What are your strengths on the bike?

8. What are your weaknesses? Do you crack on long climbs? Have trouble getting the speed up during time trials? Don't have the power in a hard sprint? Don't have the endurance to pound out a century?

9. What are you looking to improve by training with Elite Endurance? Do you have an overall training objective?

10. How many hours a week can you devote to training? What is your work schedule like?

11. What are your "A" priority events? Most athletes have two events they plan to "peak" for, and at these goal events you want to be 100% prepared.

12. What is your favorite type of riding, and what do you enjoy most about the bike?
